Enroll in Our Patient Portal

If you have been seen at one of our hospitals in Beaumont, Corpus Christi, New Braunfels, San Antonio in Texas or Lake Charles, Alexandria, Shreveport in Louisiana and are unable to self-enroll for a patient portal account please fill out the form below.

The security of your data is important to us. If any information you submitted does not match our records, your request for Patient Portal access may not be able to be completed. In these instances, to request your health data, you can visit the Health Information Management department in person at the facility where you had your visit.

Please allow 72 business hours for your request to be reviewed and completed. If you do not receive an email within 72 business hours, our support team can be reached at 855-344-0442.
